Job Details Job Location: Anacortes , WA 98221 Position Type: Full Time Education Level: Graduate Degree Salary Range: $55.00 - $62.00 Hourly Job Shift: Day Qualifications Master’s Degree in Speech Language Pathology from an accredited program. Certificate of Clinical Competency or eligibility for a Clinical Fellowship Year. Assumes responsibility for ongoing continuing education and professional development. Essential Job Function Provides speech therapy services to residents and initiates referrals when appropriate and in accordance with the Therapy Department Procedures. Conducts screening of residents at regular intervals to determine need for intervention / treatment. Evaluates residents to obtain data necessary for treatment planning and implementation. Conducts specialized evaluations as indicated. Interprets and communicates evaluation findings to residents, families, and team members. • Develops treatment plans by identifying a target list, including short and long-term goals and methods to achieve identified goals. Coordinate’s treatment plan with resident, family and other team members. Implements and conducts treatment as outlined in treatment plan. Monitors resident’s response to intervention and modifies treatment as indicated to attain goals in accordance with physician orders. Develops appropriate home or programming within the community to maintain and enhance the performance of the resident in their own environment. Documents results of resident’s assessment, treatment, follow-up, and termination of services. Adapts treatment to meet the individual needs of the resident. Provides, recommends, fabricates adaptive devices, or other equipment. Trains resident, family, and appropriate staff in the use or application of orthotics and prosthetics. Instructs resident, family, and staff in appropriate speech-language procedures or home program to allow carry over of therapeutic remediation and goals in daily activities. • Participates in discharge planning. Reviews the quality and appropriateness of the total services delivered and individual occupational therapy programs for effectiveness and efficiency, using pre-determined criteria. Participates in resident, family, and staff education. Confers formally and informally with other team members in coordinating the total rehabilitation program of the resident. Adheres to Speech Therapy Department procedures regarding documentation of speech therapy services. Provides in-service education to the members of the resident’s care team and education to the community. Provides treatment in accordance with established standards of practice, department procedures, and productivity standards. Consistently demonstrates sound judgement in the evaluation, planning, implementation, and follow-up of resident therapy programs. Reports to work on time and coordinates schedule to achieve maximum productivity and efficiency during assigned shift. Assists with and participates in Quality Assurance Program as required. Maintains a current knowledge of law and regulations regarding rehabilitative services and consults with supervisor to ensure that all employment actions are incompliance with applicable regulations. Attends and participates in scheduled trainings, educational classes and meetings required for requested for position. Ensures a safe environment and utilizes tools and equipment in a safe manner complying with all infection control, universal precautions, and OSHA standards for the healthcare personnel. Reports all hazardous conditions including incidents, injuries and equipment to Supervisor/ appropriate personnel immediately. Responds to and acts appropriately in emergency or disaster situations. Adheres to high standards of cleanliness, grooming, hygiene and dress code. Adheres to all policies of company including job description, certification requirements, Mission Statement, HIPPA, confidentiality standards and residents rights. Adapts to changes in responsibility in a positive manner, is receptive to new ways of completing tasks and is flexible in carrying out assignments. Communicates and interacts professionally and effectively with residents and coworkers throughout the community ensuring a cooperative environment while maintaining self control Communicates and interacts professionally and effectively with family members in attending to and meeting their requests.
