Job Title: Adult Senior Practitioner – Multidisciplinary Community Social Care Team Location: London Borough of Newham Council Rate: £32 per hour An exciting job role has recently become available at the London Borough of Newham Council. They are looking for a new member for their multidisciplinary community social care team. Person-Centred Support Planning: Develop support plans tailored to the specific needs and preferences of individuals. Long-Term Support Arrangements: Establish and manage long-term support for individuals with complex needs. Mental Capacity Assessments: Perform assessments to determine individuals’ capacity to make decisions. Court of Protection Work: Engage in legal processes to protect individuals who lack the capacity to make certain decisions.
