As a member of this interdisciplinary healthcare team, the Social Worker collaborates to provide pre and post operative care to adult patients. Additionally, the Social Worker supports the specialized day-to-day needs of patients through the provision of psycho-social screening and assessments; individual and family counseling and treatment; crisis intervention; bereavement counseling; liaison and referral to community agencies; advocacy and discharge planning. 1. Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a Social Worker 2. Baccalaureate degree in Social Work from an accredited School of Social Work; Masters degree in Social Work Preferred 3. Demonstrated 2 years recent experience within the last 5 years as a Social Worker in an acute adult surgical unit 3. Knowledge of Healthcare Consent Act, Substitute Decisions Act, Child and Family Services Act 4. Excellent knowledge of community support and placement services 5. Excellent crisis intervention skills 6. Demonstrated ability to work as a member of interdisciplinary healthcare teams 7. Demonstrated ability to organize and prioritize patient caseload to work in a fast paced, high turnover environment 8. Excellent communication skills 9. Evidence of commitment to professional practice and ongoing professional development
