The Registered Practical Nurse working in the Virtual Nursing Station-Surgical Transitions program -provides patient care in accordance with the telepractice standards of The College of Nurses of Ontario, and HHS standards, policies and procedures. The Registered Practical Nurse will collaborate with an interdisciplinary team and will be responsible for all preparation required to conduct virtual visits and follow up with all patients receiving remote symptom monitoring including scheduling study appointments and following up with them by video or phone. Ambassador of the virtual surgical transitions program, and visiting patients & families in hospital to provide program information. 1. Current certificate of registration with the College of Nurses of Ontario as a Registered Practical Nurse and eligible to practice as a RPN in Ontario. 2. Minimum 2 years nursing experience in a hospital/outpatient setting. 3. Recent (within 2 years) nursing experience (outpatient surgical setting preferred) 4. Current certificates in – Physical Assessment and Health History Taking (or equivalent) 5. Recent (within 2 years) patient assessment course or related experience demonstrating competent patient assessment skills and ability in health history taking. 6. Demonstrates critical thinking and decision making in assessment and problem identification and care planning. 7. Demonstrated ability to communicate (written and verbal) effectively with patients, families, and other members of the healthcare team. 8. Demonstrated skill and ability in management and triage of patient referrals 9. Demonstrated ability to function within an interprofessional team, including nursing collaboration with RNs and other members of the healthcare team. 10. Demonstrated ability to assess concurrent demands, set priorities, organize work, and maintain accuracy and concentration in periods of high activity in response to patient and team needs. 11. Demonstrates ongoing personal professional education and development 12. Demonstrated computer proficiency and experience with EPIC, MOSAIQ, and MS Office. 13. Experience with EPIC workflows and EMR best practices preferred 14. Previous experience and knowledge of virtual care preferred
