We are currently expanding our Homecare Clinical team, and we are looking for an Integrated Registered Nurse who will work cross-functionally (Homecare, Community Nursing and Clinical Trials) across Nelson and surrounding areas. About the Position | Te Mahi We are looking for a compassionate and highly skilled Registered Nurse to join our team in a full-time, permanent role (40 hours per week, rostered Monday to Sunday). This is an autonomous and a rewarding position, where you’ll visit patients in their homes, applying your practical nursing expertise and clinical assessment skills. The successful candidate will provide patient education, case management, and build strong relationships with GP practices and other referrers. We’re seeking someone experienced, clinically confident, and motivated with excellent wound care knowledge, ideally gained through community or district nursing. Key responsibilities: Conduct hands-on service user assessments and Community Support Worker training and competencies. Monitor progress and review service user goals on a regular basis Empower the people we support to live their best lives through a client-centred, holistic approach. Report incidents and escalate any concerns in a timely manner to the Clinical Lead. Collaborate and coordinate with Client Service Managers and the Homecare Clinical Lead to deliver outstanding community services. Apply strong clinical skills in medication management, enteral feeding, falls and pressure injury prevention, and wound care, while assisting in clinical trial activities and expanding knowledge in research-based care. Communicate effectively with primary and secondary healthcare providers; knowledge of ACC and MOH community contracts is an advantage.
