Job Title: Social Worker – Older Adults Mental Health Team Location: Waltham Forest Rate: £32 per hour An exciting job role has recently become available at Waltham Forest. They are looking for a new member for their Older Adults Mental Health Team. Responsibilities: You will be working with people with functional and organic (dementia) mental health diagnoses, practising as a care coordinator in a multi-disciplinary team setting. You will carry a caseload of 18-20 cases, monitoring and supporting our clients' mental wellbeing. Your role will include completing and implementing Care Act 2014 assessments and support plans, Mental Capacity Assessments, and safeguarding enquiries. Familiarity with Rio and Mosaic systems is essential.
