ESSENTIAL FUNCTIONS:

  • Conducts comprehensive assessments to establish individuals’ strengths, limitations and treatment plans to improve their well-being, health, and quality of life. 
  • Develop the appropriate therapy treatment intervention based on the needs of the individual, in addition report progress of long- and short-term treatment goals.  
  • Introduce new skills and approaches for individuals to achieve in daily life. 
  • Evaluate individuals’ living space and/or other environment to ensure treatment plans meet safety protocol as well as consistent with the individuals’ resources.
  • Provides support, counsel, advice, training and education for individuals, family, caregivers and other professionals on the importance of being consistent with therapy treatment plans.
  • Efficiently assembles extensive documentation on individuals’ progress, care services and therapy adjustments, when necessary, that aligns with standard medical documentation and facility protocol/procedures documentation.
  • Collaboratively work with other healthcare professionals to provide comprehensive care. 
  • Must display excellent active listening skills, critical thinking skills, verbal/written communication skills, time management skills. 
  • Must display compassion, integrity and respect to all individuals.
  • Supporting additional responsibilities and/or tasks as needed for the department. 

PHYSICAL REQUIREMENTS:

  • Physically, be able to lift, transfer and assist patients while providing therapy interventions during physical activity. 
  • Must be able to lift and carry items up to 25lbs, including assisting individuals when needed.
  • Ability to visually be able to perform job duties, including examining individuals and reading information from printed sources and computer screens. 

OTHER REQUIREMENTS:

  • Education: Must be a graduate of an accredited Occupational Therapy program. 
  • Licenses/Certifications: Must hold an active and in good standing Wisconsin Occupational Therapist License and CPR Certification.
  • Follows/adheres to organization, federal, and state regulations and guidelines, including Resident/Client rights
  • Maintains neat, well-groomed appearance, adhering to dress code of organization/program
