Job Title: Occupational Therapist – ACE Team Location: Rhondda Cynon Taf (Hybrid – Community & Remote) Pay Rate: £31.46/hour Working Hours: Full-time, 37 hours/week Contract Type: Locum full time Job Description: Liquid Personnel is seeking an experienced Occupational Therapist to join its client’s ACE Team based in Rhondda Cynon Taf. This role is part of a progressive and forward-thinking Adult Social Care service in Rhondda Cynon Taf. The team is committed to delivering rapid-response support to vulnerable adults 365 days a year through the Stay Well@Home 2 service. As a Locum Occupational Therapist, you will play a vital role in this mission and participate in a rota covering this essential service. What will your responsibilities be? Accessing Services Provide advice, information, and signposting Assess urgency of referrals and initiate enquiries Safeguard vulnerable individuals and ensure accurate record-keeping Transfer information to partner agencies in a timely manner Assessment of Need Conduct holistic, person-centred assessments Collaborate with individuals and carers to agree personal outcomes Explore creative solutions to maximise independence and choice Arrange equipment, adaptations, and inter-agency support Record unmet needs and complete documentation in line with statutory guidance Attend case conferences and inter-agency meetings Prepare specialist reports, including court documentation when required Requirements: HCPC registration Occupational Therapy Degree Experience in adult social care and community-based work Willingness to participate in a 365-day rota Ability to work independently and flexibly Access to a vehicle is desirable This is a hybrid role, with most work done remotely or in the community.
