This is a fixed term contract until December 2026. *Please note that access to your own vehicle is essential, as you will need to travel across the Norfolk area.* Who we are Reed Wellbeing is a public services provider that aims to support people in making sustainable lifestyle changes to improve their physical and mental wellbeing. Since 2015, we have been delivering a range of evidence-based, person centred interventions and services, including: NHS Health Checks, Smoking Cessation, Weight Management and Type 2 Diabetes Prevention and Treatment. Our NHS Health Check Catch-Up programme is a local preventative service for adults aged 40 – 74. It is designed to identify early signs of conditions such as heart disease, diabetes, kidney disease, stroke, and dementia. What is the role about? Our Health Check Practitioners are responsible for delivering our NHS Health Check service to eligible residents across Norfolk in community settings such as workplaces, libraries, and public venues. This will involve conducting and assessing lifestyle questionnaires, physical measurements, and taking bloods and blood pressure measurements. Most importantly, you’ll also be motivating individuals to make positive lifestyle changes that support long-term health and wellbeing. All Practitioners will complete comprehensive training in order to carry out the NHS Health Check programme which will include e-learning, lifestyle advice training, group and one-to-one training sessions in carrying out testing, understanding local services, procedures and information governance. Just some of your day-to-day responsibilities will include: Taking Ownership · Setting up health check workstations at various venues, ensuring participant privacy and safety is maintained · Conducting NHS Health Check questionnaires, as well as Point of Care testing such as finger prick blood testing and blood pressure measurements in line with Reed Wellbeing’s policy and procedures · Capturing results accurately and confidentially on Reed Wellbeing systems Being Fair, Open, and Honest · Advising participants of their test results and risk of cardiovascular disease, and signposting them to specialist support if required Working Together · Promoting the service across community venues and distributing marketing literature to increase awareness What’s in it for you? To be successful in this role, we are looking for someone with: · The ability to establish an effective and appropriate rapport with programme participants who need to achieve and maintain lifestyle changes (e.g. healthy weight, increased physical activity, reduction in alcohol intake). · An understanding of and/or interest in health-related physical activity, healthy eating and health related behaviour change. · The ability to coach individuals and groups to identify solutions and develop knowledge and skills to support behavioural change. · Demonstrable time management and personal organisation skills with the capacity to monitor, evaluate and report on work carried out. · Willingness to undertake evening and weekend work as required. · Willingness and flexibility to travel in order to deliver programmes across designated region. Desirable Criteria: · Experience of health check delivery. · Experience in using coaching skills to help individuals to improve their health and/or physical fitness. · Fluency in community languages. Community languages may include: Cantonese, Mandarin, Hindi, Urdu, Punjabi, Somali, Vietnamese, Dari, Yoruba, Turkish, French, Spanish, Albanian, Arabic, Bengali, Farsi, Guajarati, Polish, Serbian and Greek (dependent on location of service delivery). · An understanding of current guidelines and good practice for health-related physical activity, healthy eating and health-related behaviour change.
