The right candidate will join a team of professionals to support and mentor individuals to enhance and develop independent life skills. Communitas offers a stellar benefits package that includes three weeks of vacation for new employees, one week of sick time, eleven holidays, employer paid life insurance and long-term disability, medical and dental insurance, and more! 40 hours per week, Monday – Friday, 8:00am – 4:00pm Participates in the development and direct implementation of curriculum to teach groups in both a classroom setting and out in the community. Provides personal care and assistance to individuals consistent with both their physical needs and defined support plans. This includes complete familiarity with the profiles and histories of individuals supported and active indications that care and assistance follows the prescribed needs. Participates in the development and direct implementation of the Individual Support Plan, write annual assessments and goals and attend team meetings as scheduled. Develops and implements community-based activities that promote personal choice, community inclusion, dignity, and respect while enhancing personal skill development. Implements clinical and behavioral treatment plans, record target behaviors on data sheets and participate in clinical consultation as needed. Assesses individual’s training needs, develops descriptive teaching programs and curricula and monitors effectiveness through data collection. Qualifications • High school diploma required, Bachelor’s Degree in human services setting preferred. At least one year of directly related experience working with adults with developmental disabilities. • Valid Driver license, acceptable driving record and reliable transportation. • Acceptable CORI report and suitable fingerprint results required. • Adequate skills and ability to use computer for documentation, email communication and on-line training.
