Job Description As a Community Care Professional, you will provide essential care and support to clients in the community —enabling clients to maintain dignity, independence, and quality of life within their own homes; Key Responsibilities Deliver high-quality, person-centred care in clients’ homes Support with daily living tasks such as washing, dressing, mobility, light household duties and meal preparation Provide companionship and build positive, trusting relationships Assist with medication prompts and general wellbeing monitoring Accompanying your client to appointments, or a visit to the shops Record and report care activities accurately Promote independence, respect, and choice in every interaction Communicate effectively with clients, families, and team members to ensure seamless care coordination Support clients in maintaining their independence and social engagement within the community Additional Information What we provide: One hour minimum visits with clients to allow time to care, complete tasks without rushing and get to know your clients. Matching of Care Professionals and clients based on mutual interests and hobbies, so there is never something short to talk about and you can provide more personalised support.
