In addition, specific functions may change from time to time: Demonstrates standards of performance (ownership, teamwork, communication, compassion) that supports principles of service excellence Communicates with internal and external stakeholders using problem-solving skills to facilitate patient placement and transfers into and out of Cape Fear Valley Health System Receives physician orders on new admissions/transfers Monitors bed availability and coordinates bed placement, transfers, and discharges Maintains a list of patients awaiting transfer into Cape Fear Valley Health System Serves as a liaison for patients, families, physicians, and staff related to patient transfers, bed availability and patient flow issues Facilitates effective utilization of technology available for expediting patient flow through the health system Collaborates with Nursing Leadership, Care Coordination, and individual Units to facilitate patient throughput Other duties as assigned Minimum Qualifications The following qualifications, or equivalents, are the minimum requirements necessary to perform the essential functions of this job: Education and Formal Training: High school diploma or equivalent required EMS credential preferred Work Experience: Computer experience required Hospital and/or business office experience with specific focus in patient logistics preferred EPIC experience preferred Knowledge, Skills, and Abilities Required: Ability to communicate needs and concerns in a constructive and positive manner.
