Learn more at shn.ca Job Description: The Change Management Specialist will be a member of the SHN Digital Services portfolio in support of the Central East Local Delivery Group (LDG) digital services expansion mandate. The Change Management Specialist will play a key role in the successful implementation and adoption of provincial digital services. The Change Management Specialist will work to establish/grow stakeholders’ awareness of the system benefits of provincial digital tools (such as eReferral) and will support meaningful participation to ensure the local needs of clinicians and patients are incorporated into both the process and system design. The individual selected will develop and nurture relationships with clinicians and administrative staff to understand their needs and provide support in developing, implementing, and on-going operation of the eReferral solution in their practice. Responsibilities: Support stakeholder outreach and engagement activities with primary care physicians and specialists, related to provincial digital tools spread and onboarding in central east region of Ontario Health east Work with SHN leadership, Central East LDG, and Ontario Health East Region to apply a structured change management approach and methodology to support provincial digital services adoption Use knowledge base of the program to help clinicians and organizations understand the value coordinated access and provincial digital services can bring to their practices and patient experience Deliver structured presentations to increase awareness of provincial digital services activities and value-add to the system Provide on-job support and formal training to primary care clinicians, specialists, and their support staff as needed Work with the Project Manager(s) to collaborate with clinicians and organizations to identify, develop, communicate, and implement new business process strategies and objectives to incorporate provincial digital tools into their local environments Support the Ontario Health Patients Before Paperwork program and Central East LDG to develop customized readiness, on-boarding, and learning materials for local clinicians and organizations as needed Develop solutions for accessing hard to reach consumers such as solo practitioners and community based programs Maintain information exchange and collaboration with the Ontario Health Patients before Paperwork Program to support benefits realization activities and ensure local engagement complements the broader program vision Identify potential risks, points of resistance, performance gaps and develop appropriate mitigation activities Document lessons learned to be shared with the project team Create and enable reinforcement mechanisms and celebrations of successes Requirements: Completion of post-secondary education (Degree or Diploma) in Health Sciences or Business Administration, or Health Informatics Experience in project management and digital health is an asset Previous experience with enabling technologies and digital health an asset Well versed in use of virtual meeting tools (eg, Zoom, Teams), computers and basic troubleshooting in the work environment Familiarity with electronic medical record systems an asset Understanding of change management strategies and principles An understanding of clinical processes in primary care and specialist care is an asset Knowledge of the Ontario health care system and its institution Excellent relationship building, collaboration, and engagement skills, including conflict resolution, negotiation and mediation skills Ability to work effectively with a diverse group of clinical and non-clinical stakeholders in diverse organizations Ability to effectively communicate both verbally and in writing in English, bilingual in English and French will be considered an asset Demonstrated ability in delivering training for primary care and specialists on the use of digital health technologies Skilled at challenging accepted ideas and practices to promote consideration of alternate approaches Analytical and systematic approach to problem-solving A demonstrated track record of being a change agent Accommodation and Diversity Statement: Scarborough Health Network (SHN) embraces and celebrates our community’s unique multicultural heritage and diversity.
