This is a Regular Full-Time Opportunity Type: Replacement Job Description Summary of Duties, but not limited to: Patient Care Triages patients for appropriate placement in MSK program Interpret and critically analyze assessment findings to develop a plan of care and person-centered goals / evidence-based interventions Initiates select diagnostic and lab tests or patient injections through medical directives and delegated acts within defined scope of practice. Provides evidence-based interventions (treatment, exercise prescription, and education) incorporating comprehensive assessment considering comorbidities and multisystem risk factors Provides specialized care, education and counseling for select patient population(s). Uses clear communication and communication strategies respectfully and clearly to other members of the healthcare team, to enhance care coordination, collaboration and common understanding of patient care Maintains clear and concise records regarding patient care, progress, and patient care management decisions according to standards set by the Regulatory College and Women’s College Hospital standards of documentation Provides care delivery through innovative approaches such as Virtual Care Works to full scope of practice identifying where directives are required for optimal patient care, and leads the development and implementation of medical directives Academic Activities Supervise, train and mentor learners in clinical placements, teach small group learner educations sessions and provide other teaching as requested and supported by the program Eligible for status appointment at the University of Toronto Communicates progress and facilitates support of learners in difficulty with the University May represent WCH on education/learner-related committees (internally and externally) Ensures patient care is based on best available evidence. Dissemination of quality improvement/evaluation/research initiatives through knowledge translation Administration and Management Collaborates with other program members regarding selection and development of program design and improvement Provides guidance to administrative staff regarding referrals/booking management Professionalism Adheres to the code of ethics and practice standards for the profession Engages in self-reflection, continuing education and professional development for ongoing competence and skills Provides education to medical, nursing and health disciplines and as part of interprofessional rounds (departmental and inter-departmental) or other such opportunities Serve as a public and professional liaison for community outreach and education. This may include liaison with media, other health care providers or community agencies Acts as a clinical mentor and role model for staff and members of the health care team. Qualifications Master’s degree in Physiotherapy or Occupational Therapy, required Registered and in good standing with a Regulated Health Profession College of Ontario Must have successfully completed the Advanced Care Practitioner in Arthritis Care Credentialing Program Five years clinical practice, at least 2 years must be musculoskeletal experience Rheumatology experience is preferred Experience in an academic, outpatient environment, preferred Experience working in interprofessional teams, working closely with health disciplines, medical and nursing staff Familiarity with evidence-based guidelines and tools, required Advanced ability to provide patient and family education Advanced clinical skills (e.g. therapeutic rapport, patient assessment, clinical reasoning, etc.) Experience working under medical directives and delegated acts Experience as a preceptor an asset Demonstrated teaching ability, and leadership skills to facilitate meetings, volunteers and students, required Advance application of clinical reasoning and implementing interventions that are evidence-based Excellent organizational and time management skills Ability to conduct quality improvement/evaluation/ research Advanced knowledge of core competencies of professional standards, ethics and reflective practice Excellent verbal, written and interpersonal communication skills Excellent problem solving, critical thinking, decision-making and conflict resolution skills, required Proficiency in computer skills, specifically in Microsoft Office applications and electronic medical records, required Good work and attendance record required Demonstrated WCH vision mission and values Professional behaviour and communication that meets the standards of the professional regulatory college or association, as applicable, and the standards of Women’s College Hospital This position plays a critical role in acting as an advocate for safety and will demonstrate principles, practices and processes that will optimize a safe environment for all Why Choose Us?
