Band 6 Occupational Therapist – Discharge Planning Location: Barrow-in-Furness Contract: Temporary Our client is seeking an experienced Band 6 Occupational Therapist to join their team in Barrow-in-Furness. This role focuses on discharge planning, ensuring safe and timely transitions from hospital to home or community settings, and working closely with patients and the multidisciplinary team. Key Responsibilities: Lead and coordinate discharge planning for patients within an acute setting. Complete functional assessments and recommend equipment or adaptations to support safe discharge. Work collaboratively with the multidisciplinary team to facilitate smooth transitions. Promote independence and optimize patient outcomes through evidence-based interventions. Requirements: HCPC registration as an Occupational Therapist. Proven experience in discharge planning within acute or community settings. Strong understanding of discharge pathways and admission avoidance strategies. Excellent communication and teamwork skills.
