Our Short-Term Support Services provide a single point of access to a range of community-based support, including: Reablement and residential rehabilitation Support with hospital discharge and recovery Admission prevention and crisis response Discharge to Assess – ensuring assessments happen in the most appropriate environment We work collaboratively with health and social care professionals to ensure people receive timely, effective support that promotes independence and wellbeing.
