A primary function of this individual will be to hear the customers' voice and assist them in achieving their health needs through the development of strong relationships and individualized holistic care plans. Working in a multidisciplinary environment including, but not limited to Nurse Practitioners, Physicians, Public Health Nurses, Community Outreach Workers, Nutritionists, Addiction Workers, Social Workers and other professionals and agencies within the community. The incumbent is expected to employ an effective combination of RQHR goals combined with guidance from colleagues, personal experience, post-secondary education and specialized training to assist patients in their efforts to sustain balance in all aspects of their lives. Human Resources Exemption: No Education Bachelor of Social Work (BSW) Experience Bachelor of social work and registration with SASW required. Experience working in partnership with the Aboriginal community. Experience in assessment techniques appropriate to an chronic pain population. Experience with and demonstrated knowledge of internal and external stakeholders. Previous experience working with clients that have complex medical and non-medical needs. Knowledge, Skills and Abilities Knowledge of factors affecting vulnerable populations including poverty, family violence and the cycle of abuse, addictions and substance abuse, mental health issues, social isolation and racism. Knowledge of family systems, including aboriginal family systems and the process of cultural recovery and healing. Knowledge of assessment and treatment techniques appropriate to an aboriginal/multicultural population. Knowledge of aboriginal culture, values and traditions and their impact on accessing mental health/alcohol and drug services. Knowledge of factors contributing to mental health during pregnancy, postpartum period and parenting. Knowledge and demonstrated understanding of First Nations and Métis culture and spiritual values. Knowledge of human behaviour models and models of behaviour change, and skill in applying them to affect change. Excellent communication, writing, presentation and interpersonal skills. Crisis management and diffusion skills. Strong listening and assessment skills. Knowledge of all levels of government (federal, provincial, Band, local, etc.) and the services they provide. Understanding of the Ministry of Social Services policies, procedures and regulations. Knowledge of the Mental Health Act and Health Information Protection Act Brief treatment modalities (MI, CBT, BI). Knowledge of Health Canada’s Indian Health Benefits unit medical directives. Knowledge of current services and programs available within SHA (f/RQHR). A clear understanding for what is beyond your professional role and ability to set boundaries and maintain a professional relationship with patients. Self-directed and able to organize and manage multiple tasks simultaneously. Ability to promote and build teamwork and multidisciplinary care concept. Ability to demonstrate non-judgmental care and service to wide ranging client groups including those who may exhibit unstable behaviour. Maintain communication to locate clients who are “at-risk” for barriers to accessing services to improve their health. Promote client well-being and coach clients to become advocates for their own care. Other Information Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement. HSAS seniority within the Saskatchewan Health Authority will be considered under the Selection Criteria. Scheduled hours of work may be reduced (if applicable) during the rotation stipulated when a statutory holiday is recognized during that period. This is a community based position and may require providing services in a variety of settings including an office, primary care clinic sites, other clinics, community meeting places, and/or within a private residence including home visits. Home base for this position is: 1056 Albert Street. Must have current practicing registration with the Saskatchewan Association of Social Workers (SASW). This is a field hours position. Hours may vary based on client needs. Must possess a valid Saskatchewan driver’s license and have access to a vehicle. May be responsible for transporting clients. May be required to provide services in a variety of settings including an office, primary care clinic sites, other clinics, community meeting places, and/or within a private residence including home visits.
