Your role and responsibilities As an Independence and Wellbeing Practitioner, you will play a key role in supporting adults to achieve and maintain their independence and wellbeing. You will: work directly with individuals to carry out asset-based assessments and reviews, identifying personalised solutions that promote independence and quality of life encourage creative thinking and solution-finding to ensure good outcomes for the people we work with act as a trusted assessor, providing expert advice on daily living equipment, adaptations, assistive technology, and digital care solutions, with a strong focus on reablement develop, implement, and review enablement and support plans, ensuring each customer receives tailored support that meets their unique needs and circumstances offer clear financial and benefit advice, working within budgetary guidelines to ensure value for money for both customers and the service empower customers and carers by advising them on their rights, available options, and support networks, enabling informed decision-making about their future collaborate closely with social work teams, health professionals, and voluntary sector partners to deliver integrated, joined-up care maintain accurate and timely records, contributing to departmental and government performance requirements share knowledge and best practice with colleagues, supporting team development and participating in team duties such as rotas, meetings, and training. You will need strong communication skills to build positive relationships with customers, families, advocates, and professionals the flexibility to respond to urgent situations and contribute to team priorities as they arise resilience and a positive, solution-focused attitude when working under pressure.
